For those who're interested in taking part in the Blood Donation Drive on 19 February 2009, please note the following details and obtain the consent form from me(karen).
Date: 19 February 2009 (Thurs)
Time: 9.30am to 3.30pm
Venue: Reading Room (Block A Level 3)
Important information from the first aid club
Students who have not turned 18 yrs old by 19 feb 09 will need parental consent to donate blood. Please complete the forms and get them signed by your parents/guardians.
Interested student must be at least 16 yrs old on 19 feb 09, weigh at least 45kg, should not display symptoms of flu/sore throat, should not be on any medication for 3 days prior to blood donation.
Interested students to bring signed consent forms on the day of blood drive, 19 feb 09. Remember to bring along your NRIC or other photo identification showing your Date of Birth.
Students are not allowed to skip lectures/tutorials/practicals for the purpose of blood donation.
